Founded in 1987 by a small group of physicians who believed medicine should put the patient at the center, Meridian Health Group has grown into a multi-specialty practice serving more than 4,200 patients across three Bay Area locations.

From a single Mission District office to a regional medical group
In 1987, Drs. Eleanor Park and Robert Chen opened a two-room family medicine practice on Valencia Street with a hand-painted sign and a borrowed exam table. Their goal was simple: provide the kind of care they would want for their own families.
By the mid-1990s, demand had outgrown the original office. Meridian moved into its current Downtown Medical Center, expanded into pediatrics and OB/GYN, and welcomed Dr. Sarah Chen—now our Chief Medical Officer—as one of the first new internists.
In 2008 we opened Westside Clinic to serve the Sunset District, and in 2017 the Northgate Family Practice followed in Marin County. Today, Meridian operates as a fully integrated multi-specialty group with 18 providers, a unified electronic health record, and an in-house care coordination team.
What hasn't changed since 1987: every patient has a primary provider who actually knows them, every chart is reviewed by a human, and every call gets a callback before sundown.

Dr. Chen leads Meridian's clinical strategy and oversees quality across all 18 providers. Board-certified in Internal Medicine with 22 years of experience.
Dr. Williams is a board-certified cardiologist specializing in preventive cardiology, heart failure, and echocardiography.
Dr. Patel cares for patients of all ages and is recognized for her work in diabetes and obesity management.
Dr. Rodriguez performs minimally invasive joint replacement and serves as team physician for local athletic programs.
