Terms of Service
Effective January 1, 2026
1. Acceptance of terms
These Terms of Service ("Terms") govern your access to and use of the Meridian Health Group website, patient portal, telehealth services, and any other digital services we provide (collectively, the "Services"). By using the Services you agree to these Terms.
2. Not medical advice
Content on our website—including blog articles, infographics, and educational tools—is for general informational purposes only and is not a substitute for individualized medical advice, diagnosis, or treatment from a qualified healthcare provider. Always seek the advice of your physician with any questions you may have regarding a medical condition. Never disregard professional medical advice or delay seeking it because of something you have read on the Services.
3. Emergencies
If you are experiencing a medical emergency, call 911 or go immediately to the nearest emergency department. Do not use the patient portal, contact form, or any digital service for emergencies.
4. Patient portal
The portal is provided for established Meridian patients and authorized representatives (e.g., parents of minors, healthcare proxies). You agree to keep your credentials confidential, to log out after each session, and to notify us immediately of any unauthorized access. Portal messages are typically reviewed within one business day; do not use the portal for urgent concerns.
5. Telehealth terms
Telehealth visits are subject to the limitations of the technology and require a stable internet connection, a private location, and patient consent. By initiating a telehealth visit you consent to receive care via interactive video. You may revoke this consent and request an in-person visit at any time. Telehealth may not be appropriate for all conditions; your provider will redirect you to in-person care if needed.
6. Prescriptions
Prescriptions issued through Meridian, including via telehealth, are subject to applicable federal and state law. Controlled substances require additional verification and may require an in-person visit. Prescriptions cannot be issued to individuals who are not established patients or who reside outside states where the provider is licensed.
7. Fees and billing
You are responsible for understanding your insurance benefits and for charges not covered by insurance. Estimates of cost may be requested in advance under the No Surprises Act. Unpaid balances older than 120 days may be referred to collections.
8. Cancellation policy
Appointments cancelled with less than 24 hours' notice may incur a $50 late-cancellation fee ($100 for procedures). Three consecutive no-shows may result in discharge from the practice.
9. Acceptable use
You agree not to: (a) attempt to access another patient's records; (b) introduce malware or attempt to disrupt the Services; (c) scrape or harvest information; (d) misrepresent your identity; or (e) use the Services for any unlawful purpose.
10. Intellectual property
All content on the Services—including text, graphics, logos, and software—is owned by Meridian Health Group or its licensors and is protected by intellectual property laws. You may not reproduce or distribute content without our written permission, except for your own non-commercial reference.
11. Limitation of liability
To the maximum extent permitted by law, Meridian Health Group is not liable for indirect, incidental, special, consequential, or punitive damages arising from your use of the Services. Nothing in these Terms limits liability that cannot be limited by law (including liability for gross negligence or willful misconduct).
12. Governing law
These Terms are governed by the laws of the State of California, without regard to its conflict-of-law principles. Any dispute will be resolved in the state or federal courts located in San Francisco County, California.
13. Changes
We may update these Terms from time to time. Material changes will be announced via the website and patient portal at least 30 days before taking effect.
